Bilingual Family Resource Navigator Status: Full-time, 40 hours per week; Non-Exempt Location: The Matthews House, Fort Collins, CO; hybrid option up to 20% of total work hours Pay Range: $22.50-$24.00 per hour JOB DESCRIPTION: The Family Resource Navigator is a hands-on, full-time position helping families access the resources and supports they need to strengthen stability, well-being, and self-sufficiency. This position guides families through complex social service, education, healthcare, and community systems, reduces barriers to access, and provides coordinated, strengths-based navigation. By building trusting relationships and working closely with internal programs and community partners, the Family Resource Navigator helps families connect to timely, relevant, and sustainable supports that promote long-term success. QUALIFICATIONS: An associate degree in Criminal Justice or Social Work is required; a bachelor’s degree in Psychology, Social Work, Child Development, Criminal Justice, Sociology, or Counseling is preferred. 1+ years of experience in family services, case management, resource navigation, or a related human services role is required. 2+ years of experience working within Family Resource Centers, county human services, or community-based family support programs is preferred. Bilingual (English/Spanish) is required. Obtaining CPR & First Aid certification is required. Intermediate knowledge of local and state benefit programs, housing resources, childcare assistance, healthcare systems, and education supports. Ability to guide families through complex eligibility and application processes and advocate for timely access to services. Strong verbal and written communication skills and ability to build trust with families and collaborate with multidisciplinary teams and community partners. Ability to manage multiple families, referrals, deadlines, documentation, and outcomes in a fast-paced, community-based environment. Reliable transportation is required; up to 50% travel may be expected. ESSENTIAL DUTIES AND MAJOR RESPONSIBILITIES Family Intake & Navigation Support – 30% • Conduct intakes, needs assessments, and goal planning with families. • Guide families through applications and eligibility processes for benefits and services such as housing support, childcare, food assistance, healthcare, and education. Care Coordination & Community Partnership – 20% • Coordinate with community-based organizations, schools, healthcare providers, and county agencies. • Make warm handoffs and participate in meetings to ensure continuity of care and aligned service delivery. Family Engagement & Education – 15% • Build trusting, strength-based relationships with families and provide education on available resources, financial literacy, parenting support, and community opportunities. • Facilitate workshops, group sessions, and family engagement activities. Documentation, Data & Compliance – 15% • Maintain accurate and timely records in Community Care Link (CCL) and other required systems. • Track outcomes, service utilization, and compliance with grant and reporting requirements. Outreach & Referral Development – 10% • Support outreach to referral partners and the broader community to increase awareness of TMH’s Family Resource Center services. • Develop and maintain referral pathways with schools, healthcare providers, and community organizations. Program & Operational Support – 10% • Assist with program planning, resource development, and special projects. • Support Family Resource Center operations, events, and coverage as needed. Assist with other duties as assigned. SCHEDULE: • Full-time, 40 hours per week, generally Monday through Friday; hours may vary based on family and program needs. • Some evenings and weekends may be required to meet the needs of families and the position. • This role is based at The Matthews House in Fort Collins and includes community-based work and travel to partner locations.
