Hospice RN Case Manager

At Central Coast VNA & Hospice (VNA), we are committed to delivering compassionate, team-driven home health, palliative, and hospice care to individuals and families across the Central Coast.  For over 74 years, VNA has provided skilled in-home services, from rehabilitation and chronic disease management to comfort care at life's end.  With a focus on dignity, respect, and individualized care plans tailored to each patient's needs.  As a nonprofit rooted in community trust, VNA upholds a mission of patient-centered care, reinvesting resources into clinical excellence, family support, and accessible community health services.  VNA is looking for a RN Case Manager.  Job Summary: The Hospice Case Manager collaborates with physicians and the interdisciplinary care team to identify, coordinate, and address the home care needs of patients, families, and caregivers.  The Hospice Case Manager provides skilled nursing care and supportive services using the nursing process, with a focus on patient comfort, safety, and qualify of life.  They refers and coordinates other disciplines and specialty services and facilitates transitions to other levels of care or services, as needed. Manages patient care in accordance with agency standards, applicable regulations, and payer requirements. Provides education and support to patients, families, and caregivers and prepares patients and families for discharge when appropriate. Status: Full-Time Shift: Monday - Friday w/ rotating weekends Education/Experience/Skills/Qualifications:  * Active Registered Nurse license in the California * Minimum of one year nursing Home Health or Hospice experience required * Knowledge of home health, hospice, palliative care, and other agency services preferred * Current CPR certification * Ability to work collaboratively with physicians, families, caregivers, and the interdisciplinary team members Special Requirements:  * Current driver's license * Auto insurance * Current CPR certification * Reliable vehicle Joining VNA means becoming part of a trusted, mission-driven organization with more than 74 years of community-based service.  We are dedicated to supporting our employees with meaningful professional growth opportunities, a collaborative environment, and a strong commitment to work/life balance.  The compensation range for this role is determined by factors such as location and required skills.
