Adult Case Coordinator

Adult Services: Case Coordinators are responsible for providing comprehensive case management and support services to individuals with mental health and/or substance use disorders living in the community. This role involves conducting assessments, developing individualized service plans, coordinating care, and advocating for clients' access to community resources. The Case Coordinator will work collaboratively with clients, families, and community partners to promote recovery, independence, and overall well-being.
