Description Where Support Turns Goals into Progress Valley’s Forensics program empowers individuals to build stability, strengthen independence, and move toward meaningful life goals. As a Case Manager I, you'll partner with clients to connect them with resources, navigate challenges, and develop the skills needed for long-term success. Through personalized support, advocacy, and evidence-based case management, you'll help clients make lasting progress on their path to recovery and well-being. Pay: Dependent on Case Manager certification status (see below) Uncertified start at $17.50 - will obtain certification within the first 120 days and then pay will be increased Certified range starts at $18.75 - pay is calculated based on years of certified experience. Schedule: Monday-Friday | 9:00am-5:00pm Benefits Highlights Compensation Our compensation program includes tenure-based service increases and performance bonuses that recognize and reward high-performing team members. On-Demand Pay gives you access to a portion of your earned wages before your scheduled payday. Time Off We offer 15 days of accrued paid time off annually (increasing by one day with each year of service), 10 paid holidays, 2 wellness days, and paid parental leave. Health & Insurance Benefits Full-time and part-time team members working 30+ hours per week are eligible for medical, dental, vision, life, and disability insurance, as well as voluntary options including accident, hospital indemnity, critical illness, financial protection, and pet insurance. Health Savings & Reimbursement Out-of-pocket medical expenses may be eligible for reimbursement through our Garner HRA — up to $2,000 for individuals and $4,000 for families. Depending on your medical plan selection, you may also contribute pre-tax dollars to a Health Savings Account (HSA), with a company match of up to $900 for individuals and $1,800 for families. Job Summary & Deliverables The Case Manager I is a certified position that provides targeted case management and psychosocial rehabilitative services as indicated by the care plan to support clients to meet their individualized goals. Certified Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities and ensuring that regulatory compliance and organizational standards are met. Provides targeted case management services as described by the Medicaid manual Supports clients in following individualized care plans and identifying case management goals Completes documentation on services provided within Valley’s standards Meets productivity expectations as assigned by supervisor Observes and collects client urine samples as necessary Transports clients to appointments as necessary Requirements Licenses/Certification Case Manager certification (post hire as needed - required within 120 days of start date) CPR certification (post hire as needed) Driving positions require a minimum age of 21 and a current driver’s license Preferred Qualifications Previous social services or behavioral health experience Bachelor’s degree in Social Services, Psychology, or related field
