Completes assessments and determines the appropriate level of services for new members served.    Shifts Available:  Full-Time | AM | Shifts: 8:00 AM - 4:30 PM | Days: Monday - Friday Expected starting wage range is $26.46 - $32.69. Telecare works with tenants to engage them in services according to their behavioral health and case management needs. Services are provided throughout the community based on preference: on-site in The Shores and Pacific Village PSH community space; in individual units; in the supportive service office; or in the community, if appropriate.
