As a Case Manager at Pyramid Healthcare, you'll help individuals navigate treatment, connect with essential resources, and build a plan for success beyond our program. From coordinating services and supporting families to developing aftercare plans, your work will help clients take meaningful steps toward lasting recovery. If you have a background in human services or direct care and want to put your experience to work in a role that makes a difference, we'd love to hear from you! What You'll Do: Coordinate client care, appointments, treatment schedules, and aftercare services. Connect clients with community resources, healthcare providers, and recovery support services. Support admissions, client and family orientations, and treatment planning. Collaborate with clinical staff and community partners to promote continuity of care. Help clients and families prepare for life after treatment through resource navigation and discharge planning. Maintain accurate documentation and support compliance with program requirements. What We're Looking For: Bachelor's degree in a human services-related field with at least 2 years of post-degree direct care experience serving the population; OR Bachelor's degree in a non-human services-related field with at least 4 years of post-degree direct care experience serving the population. Strong communication, organization, and relationship-building skills. A commitment to client confidentiality, teamwork, and supporting individuals throughout their recovery journey.
