What you'll do
- Provide individualized patient care with the nursing team, assist residents with care-plan activities, transport Elders to activities, and provide emotional support.
- Evaluate and report on care, and share reports with team members and appropriate professional staff.
What you need
- A valid NYS Certified Nursing Assistant License is required.
- One year of experience working with Elders is preferred; candidates should be able to work in a team-oriented group and use computer programs and a Medical Records Documentation system.

- Starting rate - $18.50/hr.
· $4/hr. Evening/Night Differential
· $6/hr. All Weekend Shifts Differential

Job Description:
· Provide high quality, individualizes patient care in collaboration with our nursing team
· Take initiative while assisting and maintaining patient care
· Assist the resident in activities necessary as per individualized care planning
· Transport Elders to and from activities
· Perform evaluation and reporting
· Provide resident and family with emotional support individualized care planning
· Receive/give reports to team members/team leaders to appropriate professional staff
Candidate Requirements:
· Valid NYS Certified Nursing Assistant License (CNA) -
Required
· Minimum of 1 year experience working with Elders -
Preferred
· Ability to work in a team oriented work group
· Proficient in various computer programs and Medical Records Documentation system
