Description Sangre de Cristo Community Care is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our hospice home care team in Pueblo, Colorado. As a Hospice CNA, you'll have the opportunity to provide meaningful, one-on-one care to patients in the comfort of their own homes. You'll be an important part of an interdisciplinary hospice team focused on helping patients live with comfort, dignity and quality of life. This position works Friday, Saturday and Sunday during the day. What You'll Do Working under the direction of an RN or Case Manager and following each patient's individualized Plan of Care, you will: Provide personal care including bathing, grooming, dressing and toileting Assist with mobility, transfers, positioning and ambulation Provide assistance with meals, hydration and nutritional needs Observe patients and promptly report changes in condition to the nursing team Help maintain a safe, clean and comfortable environment for patients Travel to patients' homes and other care settings throughout the assigned service area Provide compassionate support while respecting each patient's dignity, privacy and personal preferences Communicate and collaborate with nurses and other members of the interdisciplinary hospice team Build trusting relationships with patients and families during an important time in their lives We're looking for people who don't just want a job, they want their work to mean something.
