What you need
- Associates degree in nursingor human services field required.Bachelor'sdegree in social work or human services fieldpreferred.
- One or more years’ experience in a clinical settingpreferred.

- If applicable, current licensure asaLPN, RNwithin the state where the facilityprovidesservices.

Overview
PURPOSE STATEMENT:
Provide assistance to
patients and their families by developing a docu
mented continuing care plan to en
sure ongoing support in r
ecovery and or other behavioral
, mental or health care follow up after discharge from the facility

Hourly rate is negotiable depending on years of experience, education, and other factors. The general hourly pay range for this role is $15-$19.14.
Responsibilities
ESSENTIAL FUNCTIONS:
D
evelop continuing

promote long term health and relapse prevention
.
Responsible for setting discharge appointments
for patients
, including group or individual therapy,
psychologist
or psychiatrist.
Make arrangements for community resources to facilitate discharge.
Provide
all needed documentation to other providers to
facilitate
the continuum of care.
Act as a lia
i
son between referral sources,
patients
and counselors.
Ensure all patient discharge planning
documentation
is clear,
concise
and
timely
.
OTHER FUNCTIONS:
Perform other functions and tasks as assigned.
Qualifications
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
Associates degree in nursing
or human services field required

Bachelor's

One or more years’ experience in a clinical setting
