Position Summary Provides aftercare case management and support services for children and families following foster care discharge. Responsibilities include developing aftercare plans, supporting kinship caregivers, assisting youth transitioning to independent living, providing parenting education, coordinating community resources, maintaining documentation, and ensuring service continuity for at least six months post-discharge. Qualifications: High school diploma required Bachelor's degree or two years of child and family services experience preferred Valid driver's license required Key Skills: Case management Family support and advocacy Service coordination Documentation and reporting Communication and teamwork Cultural responsiveness Work Environment: Community-based role with travel required; involves office and field work supporting children, youth, and families in the foster care system.
