Position Summary The Physical Therapist provides skilled, patient-centered care in the home setting and uses clinical expertise to support mobility, safety, independence, and quality of life. This role is ideal for a licensed professional who values meaningful patient relationships, clinical autonomy, and collaboration with an interdisciplinary care team.
