What you'll do
- Conduct start-of-care comprehensive assessments and complete OASIS timely
- Draft care plans and reconcile patient medications
- Ensure orders for care are accurate and complete
- Prepare accurate, orderly, and timely documentation
- Coordinate care with patients and provide reports to the Case Manager
- Provide health care instructions to the patient/family as appropriate
What you need
- An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.
