What you'll do
- Administer medications and treatments in accordance with the PA Nurse Practice Act
- Monitor and report patient responses to medications and treatments
- Assess and educate patients and caregivers on disease management, medications, and self-care
- Assist patients with activities of daily living, promoting independence and comfort
- Observe for changes in condition and report to RN Case Manager or Clinical Manager
- Maintain timely and accurate documentation of all care provided
- Participate in coordination of care with the interdisciplinary team
What you need
- Current Pennsylvania LPN license
- A compassionate heart and a passion for helping others
- Strong communication and organizational skills
- Ability to work independently while being part of a team
- Valid driver's license and reliable transportation
- Home health experience (preferred)

What You'll Do: As a Home Health LPN, you'll play an important role in helping patients remain safe, comfortable, and independent at home.
