People. Passion. Purpose.
P3 Health Partners is a population health management group founded and led by fellow physicians who want to shift the industry’s focus from managing illness to cultivating wellness. We are health plan agnostic and work with most plans in our markets. As the conduit between you and the health plan, we want to make a difference where it counts - with your patients, their families, and the communities where we live. The mission of P3 is simple. We want to help providers and their patients embrace a new kind of care. A kind of care that gives patients the tools and resources they need to manage their long-term health and wellness.
Overall Purpose
Are you a compassionate, mission-driven medical assistant who wants to make a lasting difference for patients with complex medical needs? As a Medical Assistant at P3 Health Partners, you'll be a vital member of an interdisciplinary team that helps primary care physicians deliver exceptional, value-based care to the patients who need it most.
This is not a typical, back-to-back clinic job. Our clinic sees a small number of patients each day, and some days none come in at all. When patients are scheduled, you'll support comprehensive geriatric assessments in the office or by telemedicine: obtaining vital signs, administering geriatric assessment tools and sharing the results with the provider. On other days, you'll do the work that keeps patients healthy between visits: finding patients who are due for screenings and tests, getting their records, reaching out to them, and keeping our data accurate.
If you want a fast-paced floor with constant rooming and procedures, this probably isn't the role for you. If you like a calm, predictable day, enjoy solving puzzles in a chart, and want to build skills in quality and population health, we'd love to talk.
What You'll Do
Patient care (when patients are scheduled)
Verify patient identity using 2 unique identifiers; obtain vital signs, administer geriatric assessment tools, and record medical history and medications for review by the provider and/or pharmacist
Perform procedures within your scope (such as venipuncture, ear lavage and medication administration) and handle specimens
Prepare for visits by reviewing open care gaps before the patient arrives
Conduct visits in coordination with other P3 team members, in the office or by telemedicine
Escalate concerns you find during a visit to the designated provider or appropriate team member
Care-gap and quality work (most non-patient days)
Review patient charts and reports to find who is overdue for standard screenings, labs and preventive tests
Call outside offices, hospitals and specialists to request records, and file them correctly
Help with patient outreach: calling patients to schedule visits and close open care gaps
Support POR responsibilities [one-line description from the manager]
Data and reporting
Clean up and correct patient data in the EHR and tracking tools
Help prepare reports that show the clinic's progress on quality goals
Flag patterns or problems you notice so the team can fix them
Team, safety and compliance
Collaborate with the interdisciplinary team to support coordinated, value-based care
Perform administrative duties as needed as a member of the team
Maintain a safe, secure and healthy work environment by following standards, procedures and legal regulations
Identify supplies that need ordering, and keep equipment running by following operating instructions, troubleshooting breakdowns, performing preventive maintenance and calling for repairs
Protect patient privacy and follow all HIPAA regulations and standards for handling PHI
Participate in quality improvement initiatives, documentation audits and other activities that support clinical excellence and operational efficiency
Take ownership of new and different requests, look for ways to add value, and share ideas to advance the clinical model
Use the technology provided with confidence
Stay current on required trainings, medical record entries, meeting attendance and other requirements
Role model the P3 Health Partners core values
