Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs. Consistent schedule - Full-Time Monday – Friday, no weekends, rotational on-call-once per year on average. Responsibilities/Day in the Life: Receive new patient referrals from clients Create charts for all new patients Input medical information in new chart Help Care Coordinators with patient needs as applicable Foster relationships with their team to ensure the CareHarmony culture is positive and supportive. May be asked to assist in conducting health assessments telephonically with patients Role may require a lot of time spent on the phone Other duties as applies Required Skills/Abilities: Great attention to detail Excellent organizational skills Excellent verbal and communication skills Excellent professional presence when dealing with colleagues Excellent time management skills with a proven ability to meet critical deadlines.
