As a Nurse Care Manager (RN), you will be responsible for care management of patients with acute and chronic medical conditions and the assessment of care needs of vulnerable populations for various clients.
This is a Hybrid position at either our West Des Moines, IA office or our Oklahoma City, OK office.
This position is contingent on obtaining a new, pending contract.
