As a Cardiac Nurse Navigator , you'll play a critical role in helping patients confidently navigate their heart health journey from referral and diagnostic testing through treatment planning and follow-up care. You'll work closely with cardiologists, radiologists, advanced practice providers, and multidisciplinary teams to ensure patients receive timely care, seamless coordination, and an outstanding experience.
