What you'll do
- Serve as the first contact for patients and families with health questions.
- Support care coordination, triage, medication management, patient education, and advocacy, and collaborate with the care team to connect patients with resources.
What you need
- Serve as the first point of contact for patients and families with health questions or concerns.
- Support patients with care coordination, triage, and medication management.
- Provide education and counseling to empower patients in managing their health.
- Collaborate with providers, community health workers, and other team members to connect patients with the care and resources they need.
- Advocate for patients facing barriers—whether that’s transportation, prescriptions, or navigating the healthcare system.
- Education: an associate degree, a bachelor's degree or a postgraduate degree
