What you'll do
- Work in a home base setting with an individual that is Trach, G- Tube and Vent dependent
What you need
- Ensure the well-being of the individual.
- Provide appropriate care in accordance with personal centered plan and all physician orders.
- Provide home care nursing to an adult in home and community settings.
- Create a self-centered environment for the individual - focused on their hopes, dreams, and goals.
- Follow all care plans, ISP, and protocols.
- Ensure that the individual’s home and medical equipment are clean and safe.
- Lead for insuring individual’s health maintenance
- Excellent Documentation skills
- Proficient in G-Tube and J-Tube care and medication administration
- Ventilator experience
- Tracheotomy experience
- Use of appropriate policies and systems as required for this position.
- Compassionate, caring, and punctual
- Current unencumbered nursing license (LPN) in state of PA
- Current CPR
- Minimum 1 year LPN homecare experience
- Minimum 1 year Trach, Vent, and G Tube experience
- Graduate of an accredited nursing program
- Communication Skills (written and verbal)
- Computer skills/data entry
- Knowledge of 6400 (CLA) and 6500 (Life Sharing) PA ODP Regulations
- High degree of willingness to learn, ability to change and be flexible based on situational requirements
- Organizational Skills and Multi-tasking
- Attention to detail
- Reliable and able to meet deadlines with minimal prompting
- Person-centered thinking experience or training
- Creative

Shared Wellness is currently seeking dedicated LPNs to work in our home base setting with an individual that is Trach, G- Tube and Vent dependent. Shifts available are PT, 8am-8pm, and 8pm-8am.
