Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals. Schedule: Monday – Friday, 7:30AM – 5:00PM. Supervisor determines schedule within the hours of 7:30AM – 5:00PM. Location: 1580 W Antelope Dr. Suite 200, Layton, UT Primary Responsibilities: Provide exceptional service to all customers Responsible for adherence management for quality measures Ensure demographics including insurance information are correct in scheduling and practice management software, along with updates as necessary per Standard Work Schedule appointments, verifying correct visit type and Primary Care Provider empanelment Work closely with Providers on care for Medicare population to ensure all Medication, HEDIS measures, and Annual Wellness Exams are completed each year Work with back-office staff to ensure smooth patient flow Resolve service issues and complaints and defer to appropriate management or clinical personnel when necessary Assist co-workers and team members with duties when requested, to include but not limited to, floating to other areas And other duties as assigned What are the reasons to consider working for UnitedHealth Group?
