We are especially interested in someone who has worked in—or helped manage—a busy physician practice, specialty office, outpatient clinic or similar healthcare setting. If you have been the person who handles referrals, follows up on tests, coordinates appointments, navigates prior authorizations, communicates with physicians and keeps patients from getting lost in the process, this may be an excellent next step. Your experience makes the difference You understand how provider offices work, where delays happen and whom to call to get questions answered. What you will do • Serve as the primary local point of contact for priests who need help navigating healthcare services. • Coordinate appointments, specialty referrals, testing, follow-up visits and communications with provider practices. • Help members understand insurance requirements, prior authorization processes, medication access and other logistical barriers; work with the appropriate professionals when clinical or coverage decisions are required. • Support transitions after hospitalization, rehabilitation or emergency department visits by coordinating next steps and needed services. • Make regular outreach calls and in-person visits at parishes, homes, hospitals and other care settings. • Maintain trusted relationships with local practices and create a practical network of healthcare resources. • Document member needs and completed follow-up clearly; coordinate with Meta Care’s nurse care managers and specialty team as appropriate. • Respect each member’s privacy, dignity and independence. This position does not involve diagnosing, prescribing or independently providing clinical treatment. • Ability to travel throughout the Diocese of Rockford for in-person visits. • Commitment to serving in a manner consistent with Catholic Social Teaching and the Ethical and Religious Directives for Catholic Health Care Services. Successful completion of required background checks and screenings is required.
