Description Pediatric Transitional Nurse – Registered Nurse (RN) / Licensed Vocational Nurse (LVN) San Antonio, TX | Bilingual Preferred (Not Required) | Full Time Pay Range: LVN: $30–$32/hour RN: $35–$41/hour Make a Difference Beyond One Shift Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Vocational Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children and their families during critical transitions to home care. This role is ideal for nurses who are confident working with high-acuity patients and skilled in trach, vent, G-tube, and emergency management . Nurses in this position play a critical role supporting families, training caregivers, and ensuring safe continuity of care in the home environment. The company was founded by a nurse with a huge heart, and today we pride ourselves on our values: H eart, A dvocacy, L ove, and O utreach! #HALOS What You’ll Do Provide skilled pediatric nursing 1:1 care in patients’ homes Perform patient assessments and ongoing evaluations -Administer medications, treatments, and therapies Support hospital-to-home transitions for high-acuity patients Educate and support parents, families, and caregivers Assist with discharge support and care coordination Train and support permanent field nurses during transitions Provide first-shift and interim coverage as needed Collaborate with Clinical Care Managers and interdisciplinary teams Respond appropriately in emergency situations and changes in patient condition Qualifications Required: Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license Current CPR certification Ability to lift and assist pediatric patients as needed At least 2 years of experience with Trach care, Ventilator management, and TPN, post-graduation Home Health Care environment experience High-acuity pediatric patients Emergency response management Schedule & Travel 12 hour shifts (Days or Nights) Flexible scheduling based on patient needs and Nurse availability Must be available a minimum 1 weekend day Staffing coverage responsibilities as needed Local travel within approximately 30–40 miles Why Angels of Care: We have a full benefits package, bulleted out below.
