Pharmacy Clerk Job Description Department: Home Infusion and Specialty Pharmacy Reports to: Pharmacy Manager or designee Proposed classification: Nonexempt, hourly Position Summary The Pharmacy Clerk provides clerical, administrative, and customer service support to the home infusion and specialty pharmacy. This position supports accurate patient records, timely communication, and delivery coordination while working closely with pharmacy, nursing, intake, reimbursement, and delivery staff. Essential Duties and Responsibilities Answer telephone calls, take accurate messages, and direct patient, caregiver, provider, and staff inquiries to the appropriate team member. Enter and update patient demographics, contact information, insurance details, and administrative records in designated systems. Scan, file, and organize prescriptions, referral documents, delivery records, and other pharmacy paperwork. Route missing or incomplete documentation to the appropriate staff member. Assist with prescription information entry when assigned and permitted, subject to pharmacist review and responsibility for accuracy. Contact patients or caregivers to confirm delivery addresses, availability, and receipt of scheduled shipments. Document communications and report delivery concerns promptly. Coordinate delivery schedules, courier communications, tracking information, and proof of delivery for orders released by the pharmacist. Escalate delays or failed deliveries to pharmacy and nursing staff. Route refill requests to the pharmacy team and assist with administrative follow-up. Refer medication changes, missed doses, side effects, and therapy questions to a pharmacist or nurse. Prepare administrative delivery documents and maintain delivery and equipment tracking logs as assigned. Assist intake and reimbursement staff with collecting insurance information and routing authorization or billing inquiries. Monitor office and nonmedication supply levels, maintain organized work areas, and report supply needs. Protect patient confidentiality and follow company policies for privacy, infection prevention, workplace safety, and record retention. Report patient complaints, documentation discrepancies, potential errors, and safety concerns promptly. Complete required training and assist with quality improvement activities. Perform other assigned clerical duties within the authorized scope of the position. Scope of the Position This is a nonclinical clerical position. The Pharmacy Clerk does not compound medications, select or measure medications to fill prescriptions, package or label medication containers as part of prescription preparation, perform final prescription verification, counsel patients, or make clinical decisions. Duties requiring a pharmacist or licensed pharmacy technician must be performed by appropriately authorized personnel. Assigned pharmacy activities must follow applicable law and pharmacist supervision requirements. Qualifications High school diploma or equivalent. Previous clerical or customer service experience preferred; pharmacy, healthcare, home infusion, or specialty pharmacy experience is a plus. Accurate data entry skills and proficiency with email, office software, and electronic record systems. Strong organization, attention to detail, communication, and follow-through. Ability to manage multiple tasks, meet deadlines, and communicate professionally with patients and coworkers. Ability to maintain confidentiality and work effectively with a multidisciplinary team. Work Environment and Physical Requirements Work is performed in a pharmacy and office setting and involves computer use, telephone communication, and periods of sitting, standing, and walking. The employee must be able to perform the essential functions of the position with or without reasonable accommodation. Any lifting requirements should reflect the actual duties assigned.
