- Role: Registered Nurse (RN) - Case Manager - Comp: $64.10 per point - SOC: $160.25 - $0.67 cent mileage reimbursement rate - Schedule: Full-time with flexible scheduling to meet your needs! - Location: Field-based WE'RE FIXING HOME HEALTHCARE, AND WE'RE BUILDING OUR PRESENCE IN DETROIT Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. SPEND MORE TIME CARING FOR PATIENTS—NOT CHARTING. At Adaptive Home Health, we're excited to offer our clinicians an innovative AI-native home healthcare platform designed to simplify your day. Enjoy customized schedules, optimized routing for greater efficiency, and ambient AI scribing that automatically documents your visits while you focus on your patients. Prefer not to record during a patient visit? No problem. Our platform also includes an after-visit voice documentation option, allowing you to capture your notes afterward and leverage AI to draft your charts. By streamlining documentation and reducing after-hours charting, our technology helps you spend less time on paperwork and more time doing what you do best, providing exceptional patient care. THE MISSION OF THIS ROLE Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. OBJECTIVES 1. Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review. 2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. 4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. WHAT YOU'LL OWN - Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge). - Direct, high-acuity patient care, with a focus on wound care - Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases. - Being the face of Adaptive to patients, families, and the referral sources who feed your pod. WHO YOU ARE / HOW YOU OPERATE - Dedicated clinician: home health experience strongly preferred - High ownership: you treat your caseload like your own panel and your pod like your own business. - Open mindset: Energized by the opportunity to shape the playbook and refine SOPs - Tech-forward: You want to leverage software to accelerate the charting process, and you'll give us blunt feedback to make it better - Trust-builder: Patients, families, and referral partners believe you because you do what you say. REQUIREMENTS - Active Michigan RN license - Reliable transportation, valid driver's license, auto insurance - Home health / OASIS experience strongly preferred OUR GUARANTEE - Full-time target: ~30 productivity points/week; clinicians earn more for exceeding it. THE REST OF THE PACKAGE - $1k bonus for referrals (we’re growing)! - Benefits for full-time (medical/dental/vision, PTO) - Mileage reimbursement - Flexible scheduling to meet your needs!
