We're known to be one of the largest facilities in the Bay Area with a 258-bed capacity, making us home to top-rated healthcare professionals throughout the City of San Jose.  Location: 1601 Petersen Ave, San Jose, CA 95129 Website: https://westwoodpostacute.com/ [https://westwoodpostacute.com/] We are seeking a dedicated and compassionate Registered Nurse to join our team at a skilled nursing facility.  RESPONSIBILITIES: * Conduct thorough assessments of residents' health status and needs. * Develop and implement individualized care plans based on assessments. * Administer medications and treatments as prescribed by physicians. * Monitor residents' progress and make necessary adjustments to care plans. * Provide education to residents and families about health conditions and care options. * Collaborate effectively with interdisciplinary teams to ensure optimal care. * Document all aspects of patient care accurately and timely in electronic records. QUALIFICATIONS: * Fresh or newly  licensed RNs are welcome * Experienced RN is preferred * Experience in a skilled nursing facility/long-term care/senior care is a plus * A valid and active California license as a Registered Nurse is required * Must have an active BLS through the American Heart Association WHAT WE OFFER: We value our team members and provide a supportive and enriching work environment.
