Have support when you need it. Elara Caring is hiring an RN Case Manager to provide home health care throughout Trumbull and Mahoning counties. Bring your nursing experience to a close-knit team where you can organize your patient visits, provide individualized care, and help patients remain safe in their homes. Strong clinical and branch support. Home health experience is helpful, but not required. What You’ll Do Assess patients and develop individualized care plans. Provide skilled nursing care in patients’ homes. Teach patients and caregivers about medications, treatments, and managing their health. Coordinate care with physicians and the interdisciplinary team. Complete timely, accurate clinical documentation. What You’ll Need Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Able and willing to travel within branch/office coverage area. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations. Must be able and willing to travel 50% Associates Degree or Bachelor’s Degree in Nursing is preferred Previous Home Health or Hospice Experience is preferred Previous experience with HomeCare HomeBase is preferred Care for patients in Youngstown, Warren, Cortland, Poland, Boardman, and surrounding communities in Trumbull and Mahoning counties.
