Description

Key Duties and Responsibilities * Conduct proactive outreach and case management for vulnerable populations through multiple platforms, including telephone, in-person visits, and field-based engagement in members’ homes and community settings. * Perform in-person visits with ECM members in a variety of environments, including private residences, shelters, streets, and other community locations, to assess barriers to care, establish rapport, evaluate member needs, and facilitate access to appropriate services. * Collaborate with the ECM interdisciplinary team to coordinate care, provide health education, and connect members with community-based medical, behavioral health, and social support resources. * Review member medical records to identify care gaps, assess complex medical and social needs, and develop individualized care plans that promote member well-being and improve care coordination. * Perform clinical reviews and support care coordination activities for ECM members, including medication reconciliation, as appropriate to the scope of the position. * Maintain accurate program activity logs, case management documentation, and required reports in accordance with organizational and contractual requirements. * Meet and maintain productivity standards, documentation requirements, and clinical performance measures established by the organization and its contractual agreements. Education and Experience * 1-2 years clinical experience * Case management experience preferred * Bilingual preferred  Essential Skills and Abilities * Thrives in an unstructured, start-up environment. * Self-starter that can work independently and collaboratively, prioritize tasks and has initiative and excitement to take on unfamiliar tasks. * Advanced knowledge of word processing, graphic presentation and computer software related to specific tasks * Demonstrated excellent computer and word processing skills with special emphasis on calendaring, presentation, and spreadsheet capabilities * Working knowledge of company policies, procedures, and operations * Excellent composition, grammar, and business language skills * Excellent communication and interpersonal skills with the ability to effectively communicate with all levels of management, patients, and family members.  * Creative, flexible, well organized, resourceful, and detail-oriented * Excellent judgment in handling confidential and sensitive information * Ability to work independently, set priorities and handle multiple tasks with a high level of efficiency * Establishing and maintaining cooperative working relationships with others * Ability to work across locations and time zones Licenses/Certifications * Must have active California licenses Core Competencies * Instills trust * Customer focus * Manages ambiguity * Collaborates * Drives results To ensure the health and safety of our workforce while doing our part to protect those around us, CareConnectMD is requiring proof of full COVID vaccination for employees as a condition of employment, subject to legally recognized accommodations.
