- Conductskilled nursing visitsin patients' homes(assessment, reassessment, medication administration, IV therapy, and treatments)
- Develop and implement individualized care plans
- Educate patients and caregivers on managing conditions and medications
- Collaborate with a multidisciplinary team to ensure continuity of care
- Document visits and patient progress accurately and timely
