We offer the following to our employees: Be part of a unique healthcare company where we can help those in our communities who need our help the most Flexible hours/work-life balance (full-time or part-time available) Competitive pay Paid on a weekly basis Medical/dental/vision/life insurance Paid holidays/PTO/401(k) match Career growth opportunities Great and collaborative work environment Responsibilities What You Will Do: Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs Manage emergency situations in a prompt, precise, and professional manner Complete medication administration and treatment needs for all assigned patients Provide nursing care in accordance with applicable regulations and Company policy Prepare and maintain accurate and complete clinical and progress notes on a timely basis Qualifications Qualifications: Current, active Registered Nurse (RN) license Valid driver's license and reliable transportation At least one year of nursing experience; home health and OASIS experience preferred Knowledge of OASIS, Medicare Conditions of Participation, and home health regulatory requirements.
