What you'll do
- Complete social service assessments and progress notes for facility residents
- Coordinate discharge plans with home health agencies and medical equipment vendors
- Assist residents and family members with Medicaid applications and paperwork
- Lead care plan conferences alongside nursing leadership and therapy staff
- Address daily resident non-clinical needs and coordinate with floor staff on care updates
What you need
- Active Texas Social Work License (LSW, LMSW, LBSW, or LCSW)
- Must pass a standard criminal background check
- Must pass a drug screening
- Long-term care or skilled nursing facility experience (preferred)

Responsibilities

Compensation, Schedule & Benefits
Pay Rate: Starting at 34/hr based on experience
Schedule: Mon–Fri (Day)

Qualifications & Requirements
Active Texas Social Work License (LSW, LMSW, LBSW, or LCSW)
