Vitality Hospice & Palliative Care is seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our clinician-owned and operated hospice agency.
What you'll do
- Complete comprehensive hospice admissions and patient/family assessments
- Ensure accurate documentation and appropriate care planning
- Provide direct patient care as needed, including medication administration and treatments
- Collaborate with physicians and the interdisciplinary team to develop individualized plans of care
- Serve as a liaison between patients, families, and healthcare providers
- Educate patients and caregivers on disease progression, symptom management, and hospice services
What you need
- An active Registered Nurse (RN) license is required.
- Hospice or admissions experience is preferred but not required; skilled nursing experience is preferred.

Admissions Registered Nurse (RN) – Hospice Job Type: Full-Time | Travel Nursing Schedule: Monday – Friday | 10:00 AM – 6 PM Work Location: On the road (travel required) Job Description Vitality Hospice & Palliative Care is seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our clinician-owned and operated hospice agency. This is an excellent opportunity to work with a supportive interdisciplinary team dedicated to providing exceptional end-of-life care.
