About Us
Forward Recovery is a behavioral health treatment facility in Los Angeles providing PHP and IOP levels of care for adults with substance use and co-occurring mental health disorders. We're seeking a dedicated Case Manager to join our clinical team and support clients through every stage of their treatment.
What You'll Do
Manage a caseload of up to 25 clients across PHP and IOP levels of care
Complete timely, insurance-ready clinical documentation in Kipu EMR (case management notes, intake summaries, treatment updates)
Coordinate care with medical providers, psychiatric services, and outside agencies
Handle prior authorizations, disability paperwork, leave-of-absence documentation, etc.
Conduct intake assessments and pre-admission summaries for incoming clients
Provide individual weekly case management sessions and support clients with treatment planning, aftercare coordination, and discharge planning
Communicate with families, referral sources, and insurance providers as needed
What We're Looking For
1+ years of experience in behavioral health, substance use treatment, or a related setting (PHP/IOP/RTC experience preferred)
Working knowledge of ASAM criteria and insurance documentation standards
Experience with an EMR system (Kipu strongly preferred)
Strong written documentation skills — accuracy, clarity, and attention to deadlines
Ability to manage a full caseload independently while collaborating with a multidisciplinary team
Certification (CADC, RADT) or relevant degree
CPR/First Aid certification (or willingness to obtain)
Schedule & Compensation
Full-time, Monday–Friday (AM Position)
