What you'll do

Provide skilled, one-on-one nursing care to patients in their homes while developing and following individualized care plans.
Collaborate with interdisciplinary teams and maintain accurate documentation in compliance with regulatory requirements.

What you need

Requires a current LPN license in Colorado, current CPR certification, and a valid driver's license with a reliable vehicle.
Home health experience is preferred but not required.
Education: a professional certificate

Licensed Practical Nurse (LPN) | [Boulder, CO] Overview At Nuclear Care Partners, we provide in-home nursing care to individuals impacted by their work in the Department of Energy or Nuclear Energy programs. As a Licensed Practice Nurse (LPN), you will make a meaningful impact by delivering skilled and compassionate, one-on-one, patient centered care directly in patients' homes. In this role you will be supported by a mission-driven, nurse-led team that values your contribution and prioritizes patient dignity and outcomes. Compensation $23.17 - $33.59 based on experience. $3/hr shift differential for nights and weekends.
