What you need
- Ensure the well-being of the individual.
- Provide appropriate care in accordance with personal centered plan and all physician order.
- Provide home care nursing to adult in home and community settings.
- Create a self-centered environment for the individual - focused on their hopes, dreams, and goals.
- Follow all care plans, ISP, and protocols.
- Ensure that individual’s home and medical equipment are clean and safe.
- Lead for insuring individual’s health maintenance
- Excellent Documentation skills
- Proficient in G-Tube and J-Tube care and medication administration
- Ventilator experience
- Tracheotomy experience
- Must be able to lift/push/pull up to 50lbs
- Use of appropriate policies and systems as required for this position.
- Compassionate, caring, and punctual
- Current unencumbered nursing license (LPN) in state of PA
- Current CPR Certification
- Minimum 1 year LPN homecare experience
- Minimum 1 year Trach, Vent, and G Tube experience
- Graduate of an accredited nursing program
- Communication Skills (written and verbal)

LPN- PART TIME- SIGN ON BONUS $1250
Shared Wellness is founded on the belief that people living with intellectual and developmental disabilities are not defined by their diagnosis and, with the proper support, are able to achieve their dreams and become valued members of their communities.
Shared Wellness is currently seeking dedicated LPNs to work in our home base setting with an individual that is Trach, G- Tube and Vent dependent. Shifts are 12-hour shifts, 8am-8pm or 8pm-8am.
