We are looking for a PRN Occupational Therapist to extend our reach through telehealth. Position Summary We are seeking a licensed Physical Therapist for a consistent PRN telehealth position covering four Florida skilled nursing facilities. These communities include skilled nursing, assisted living, and memory care residents. You will work remotely with on-site clinical staff to keep residents progressing safely toward their goals. All telehealth visits are limited to evaluations, recertifications, or supervisory visits. Every telehealth visit is conducted with a licensed PTA or COTA at the facility. Required Active, unrestricted Florida Physical Therapy license Residence within one hour of one of our facilities in Wesley Chapel, Brooksville, Winter Haven, or Ocala, FL Reliable high-speed internet and a private, quiet workspace for telehealth Strong communication skills and comfort with technology About Us: Creative Health Solutions is a therapist-operated company providing Therapy services at Skilled Nursing facilities since 2001.
