Your responsibilities will include: Responding to after-hours patient needs and changes in condition Providing symptom and pain management Performing nursing assessments and determining appropriate interventions Making patient visits when clinically necessary Educating and supporting patients and families Coordinating care with physicians and the interdisciplinary hospice team Completing timely and accurate EMR documentation Ensuring continuity of care throughout your on-call week Maintaining compliance with Medicare, state, and hospice standards Who Thrives in This Role? What We're Looking For Current Pennsylvania Registered Nurse (RN) License Minimum 1 year of RN experience Hospice, Home Health, Oncology, ICU, ER, or Med-Surg experience preferred Hospice experience is preferred but not required—we're willing to train the right RN Strong assessment and critical-thinking skills Compassionate and confident communication style Ability to work independently and make sound clinical decisions Valid driver's license, reliable transportation, and current auto insurance Make a Difference—and Get Your Time Back.
