As an On-Call RN, you will play a vital role in identifying patient and family needs, coordinating care, and offering preventative measures and compassionate support. Schedule: On Call for 7 Days on and 7 Days off from Friday at 5 pm - Monday 8 am, Monday 5 pm - Friday 8 am Territory: Santa Cruz and Surrounding Areas. Provide Empathetic and Heartfelt Assessments: Conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs. This is your chance to truly connect. Maintain Accurate Documentation: Every detail is crucial in delivering high-quality care, both in the patient's home and within our agency and regulatory standards. Your attention to detail is vital to our success. Provide Support for Families: Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones. Your presence offers confidence and reassurance during this challenging time. Qualifications Hold current unencumbered license as a Registered Nurse. Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR One (1) year experience as a professional nurse within the last three (3) years. Hospice/home health experience preferred but not required. Must be computer proficient in typing and various programs, including a background in EMR. Possess and maintains current CPR certification. Pay: Approx. $135k annually with pay differentials paid hourly; may vary with hours worked. The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position.
