What you'll do

The Patient Care Coordinator processes incoming calls and answers general inquiries as part of an interdisciplinary hospice team.
They are responsible for providing information and transferring calls to triage nurses when necessary during alternate hours.

Candidates should demonstrate professionalism, collaboration, and a commitment to the hospice mission.
The role requires the ability to effectively manage communication and support the interdisciplinary team in a fast-paced environment.
