About the company
Fira Health
Hospital & health care · 2 to 10 employees
Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care. The PTA role is a core part of that mission: you help restore function, keep patients safe at home, and drive real outcomes — in partnership with a supervising PT. We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier — protecting your time.
What you'll do
- Deliver skilled home visits under a supervising PT’s plan of care, carry out treatment focused on functional progress, and communicate updates or concerns.
- Coordinate with the care team and coach patients and caregivers on practical home programs.
What you need
- Requires an active Texas PTA license in good standing, clinical judgment, communication and documentation skills, and competency in core home health PTA scope.
- Requires a current driver’s license, daily travel across the assigned territory, and ability to transport basic therapy equipment.
