Mozaic (a chapter of The Arc) is hiring for a Planning Coordinator in Penn Yan. This position will be responsible for maintaining all documentation for Life Plan Reviews, Staff Action Plans (SAP) and Individual Protective Oversight Plan (IPOP). Develops and maintains positive relationships with Care Coordination Organization staff.
EDUCATION AND EXPERIENCE:
High School diploma or equivalent required
Experience working with people with developmental disabilities preferred
Hours: Monday - Friday; 8 am-4 pm
