You are responsible for preventing adverse events from happening as well as optimizing our health service productivity and patient outcomes. Keeping our organization's mission, vision, and values at the forefront of your mind, you facilitate the delivery of top-quality, patient-centered care while going through your daily tasks. Your tasks fluctuate based on your specific client caseload and schedule. Primarily, you provide direct patient care to acute and chronically ill patients in the comfort of their homes.rnIn addition, you validate patient assessments through explicit documentation. Helping your patients maintain their personal autonomy is one of your top priorities, and you assist them with managing their own time and learning how to adapt to new situations.
