Social Services Assistant (Full-Time) Job Description Sierra Vista Post-Acute is a 120-bed skilled nursing and rehabilitation community dedicated to helping residents recover, rebuild, and regain independence. We provide short-term rehabilitation, long-term care, dementia support, and respite services in a compassionate, home-like environment. Our interdisciplinary team delivers personalized care plans and specialized therapy services designed to promote strength, dignity, comfort, and quality of life. Why Join Us? Essential Job Functions:  * Assist with the admission process, including facility tours, inquiries, and interviewing prospective residents, families, or responsible parties. * Coordinate personal care needs and appointments, including hair care, transportation, adaptive equipment, clothing, and personal items. * Maintain communication with residents’ families and attending physicians, including invitations to care plan meetings and other appropriate communications. * Document resident contacts, services, and referrals accurately in the social services log and EMR. * Assist residents and families with forms and applications, including advance directives, Medi-Cal applications, voter registration, and other required documents. * Assist with resolving resident and family concerns, including grievances and theft/loss issues, as directed. * Participate in Behavior Management programs and provide appropriate support to residents as directed. * Protect and promote residents’ rights, dignity, individuality, and confidentiality in all interactions with residents, families, and staff. * Communicate effectively with residents, families, staff, and community or government agencies, both verbally and in writing.
