Social Worker (MSW) – Per Diem Nassau County, NY Affirmed Home Care Pay: $50.00 per hour Per Diem | Potential Full-Time Opportunity Shift Monday's, Wednesday's and Friday's |10:00 am - 6:00 pm Exciting New Opportunity with Affirmed Home Care! Affirmed Home Care is seeking a compassionate and motivated Social Worker (MSW) to join our team in Nassau County. This is a per diem position with the potential for a full-time opportunity as the program grows. The Social Worker will provide rotating onsite social work coverage across three hotel-based client service locations in Syosset, Plainview, and Woodbury. The Social Worker will play an important role in supporting clients with benefits navigation, including entitlements, housing coordination, care coordination, behavioral health support, discharge and hospital coordination, and connections to community resources. The Social Worker will also assist clients with accessing food assistance programs, transportation services, and other resources that promote long-term stability and independence. Responsibilities Assist clients with SSI/SSD applications Support Medicaid enrollment and recertification Assist with SNAP applications and other public assistance programs Connect clients with food assistance programs and transportation resources Provide guidance and support with housing applications Assist with transition planning to permanent housing Collaborate with DSS case managers and other community partners Coordinate discharge planning and hospital transitions Provide mental health referrals Coordinate substance abuse treatment referrals Provide crisis intervention and immediate support services Link clients to appropriate community-based resources Advocate for client needs and help ensure access to appropriate support services Maintain appropriate documentation and follow-up with clients and community partners What We’re Looking For Master’s Degree in Social Work (MSW) 1+ year of experience in healthcare or social services; home care and substance use experience preferred Strong case management and care coordination skills Excellent communication and interpersonal skills Ability to manage multiple clients and priorities in a fast-paced environment Compassionate, patient-focused approach to care Ability to work independently while collaborating effectively with a multidisciplinary team Why Join Affirmed Home Care?
