You will advocate for resident rights, coordinate services, and help meet the psycho-social needs of our residents. The long-term care social worker is responsible for: evaluating resident psycho-social needs completing a comprehensive assessment and developing a plan of care exercising independent judgment / making sound decisions /exhibiting flexibility when needed working knowledge of federal and state regulations ensuring and providing therapeutic interventions to assist residents in coping with their transition and adjustment to long-term care Required Qualifications: bachelor's degree in social work or related human services field current certification in the State of Wisconsin as a certified social worker, and must maintain CEU's as per WI State Statute Schedule Details: full-time: M-F, 8:00am-4:40pm no weekends or holidays More Than Just a Job - Great Benefits!
