What you'll do

Provide skilled nursing care and administer prescribed medications and treatments
Perform G-tube/PEG care, tracheostomy care, and suctioning as required
Monitor patient conditions and report changes promptly
Provide ventilator support when required and appropriately trained
Educate patients and caregivers
Maintain accurate documentation

What you need

Active New York State LPN license
Minimum of 6 months of home care experience
Current CPR/BLS certification
Strong clinical and communication skills
Compassionate, reliable, and able to work independently
Ability to meet physical requirements, including lifting up to 50 pounds as needed
