What you'll do
- Conduct Start of Care comprehensive assessments and complete OASIS timely
- Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete
- Prepare accurate, orderly, and timely documentation
- Coordinate care with patient and provide proper reports to Case Manager
- Provide health care instructions to the patient/family as appropriate per assessment and plan
What you need
- An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.
