More about our team  Join our team in a 90-bed Skilled Nursing & Rehabilitation Unit, with 20 Rehab beds and 70 LTC beds. Staffed with Registered Nurses (RNs), Licensed Practical Nurses (LPNS)  Certified Nursing Assistants (CNAs), and Medication Aides. Operating with a patient ratio of 1:15 days, 1:19 evenings and 1:25 nights, our unit cares for patients with Congestive Heart Failure (CHF), COPD, Diabetes, ESRD, Renal Failure, Dementia, Stroke, and Heart Disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs. How you’ll contribute  A Licensed Practical Nurse (LPN), who excels in this role: •    Accurately performs patient assessments and identifies patient needs •    Identifies and initiates appropriate nursing interventions •    Provides care appropriate to condition and age of the patient •    Performs timely and appropriate documentation relating to medical necessity in the medical record •    Responsible for completion and revision of the Interdisciplinary Care Plan for each patient •    Performs timely and accurate QI assessments Why join us We believe that investing in our employees is the first step to providing excellent patient care. Applicants should have a current state LPN license and possess a  degree from an accredited nursing school.
