Social Worker | Regional Float

Join the inpatient care management team as a regional float social worker. This unique role will have the ability to support multiple CentraCare hospitals as a float social worker. Our Socials Workers serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You'll collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary team of nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources. Schedule Full-time | 80 hours every two weeks | Onsite Day shift | Monday through Friday | 7:30am-4:00pm Pay & Benefits Pay based on experience and degree Bachelor’s in Social Work starting at $32.09 per hour Master’s in Social Work starting at $33.15 per hour Pay range: $30.65 - $49.73 per hour $3,000 hiring bonus and $5,000 relocation bonus for eligible candidates! Full time benefits : medical, dental, PTO, retirement, employee discounts and more! Qualifications Current Licensed Social Worker (LSW) in Minnesota required. Bachelor's or Master's degree in Social Work required. 2 years or more of clinical experience as a Social Worker. A valid driver's licensed and a driving record in good standing required. American Case Management Association (ACMA) Case Manager-SW Certification required within five years. To be eligible for the hiring and relocation bonus you must be an external candidate who is hired into an eligible full or part time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire. To be eligible for the relocation bonus, you must currently reside more than 50 miles away from St. Cloud Hospital and relocate to within 30 miles of it.
